Who Can Apply
This position is suitable for candidates who meet the following qualifications:
- Bachelor’s Degree or Diploma in Clinical Management, Nursing, Social Work, Counselling, Psychology, or related fields.
- At least three years of experience in case management for community-based HIV services.
- Experience working with both literate and non-literate audiences using participatory methods.
- Strong ability to handle program data with a high level of integrity.
Additionally, candidates with experience in Key Population (KP) programs and those who have supervised field teams will have an added advantage.
Key Responsibilities
The Case Manager will play a central role in coordinating HIV care and treatment support activities. Major responsibilities include:
- Coordinating with district HIV coordinators, health facilities, and peer navigators for client enrollment and service delivery.
- Monitoring linkage of newly diagnosed HIV-positive clients to Care and Treatment Centres.
- Managing client appointments, ART refill schedules, and follow-ups for missed appointments.
- Supporting community-based ART services and facilitating the return to care for clients who are lost to follow-up.
- Supervising and mentoring peer navigators to strengthen service delivery and case tracking.
- Compiling and reconciling case management data and maintaining regional tracking registers.
- Conducting regular site visits and monitoring case outcomes across service delivery points.
- Preparing weekly and monthly reports on case management progress, referrals, and success stories.
Skills and Competencies
Successful candidates should demonstrate:
- Strong communication skills in Kiswahili and English.
- Good teamwork and interpersonal skills.
- Basic computer skills, including Microsoft Word and Excel.
- Ability to work independently in community settings and coordinate with multiple stakeholders.
